Young adults and adolescents transition project

What we are doing

As part of the Future Hospital Programme, the Young adults and adolescents transition project aims to improve the quality of care for young adults and adolescents with long-term and complex conditions as they transition from paediatric to adult services.

The project reviewed the current state of transition services within adult physician specialties, raise awareness of the issues related to caring for young adult and adolescent patients, and support RCP members and fellows to deliver high-quality, developmentally-appropriate care for these patients.
